إجراءات تثبيت موقع الأسنان بعد الانتهاء من المعالجة بالحاصرات التقويمية

لا توجد أدلة كافية حول فعالية أي من نوع من المثبتات السنية لمختلفة المستعملة لتثبيت الأسنان بعد الانتهاء من المعالجة التقويمية بالحاصرات.
إن التثبيت  هو مرحلة من مراحل المعالجة التقويمية التي تنشد الحفاظ على موقع الأسنان المعدّل تقويمياً  بعد استعمال الحاصرات، حيث تميل الأسنان للعودة إلى وضعها السابق (النكس) في حال غياب هذه المرحلة، ولذلك لابد من تطبيق إحدى طرائق التثبيت لكل مريض أجرى معالجة تقويمية لتجنب نكس المعالجة.
لا توجد أدلة قوية في هذا المجال لتشكل قاعدة للممارسة السريرية.
وجدت هذه المراجعة دليلاً  على أن الإجراء الجراحي البسيط المرافق لاستخدام إحدى طرائق التثبيت يعد أفضل من استخدام إحدى طرائق التثبيت فقط في الحفاظ على موضع الأسنان بعد إزالة الحاصرات التقويمية ولكن كان هذا الدليل ضعيفاً ولا يمكن الاعتماد عليه. هناك حاجة ماسة لإجراء تجارب سريرية عشوائية مضبوطة عالية الجودة حول هذه المرحلة الحاسمة من المعالجة التقويمية.

الملخص:

خلفية البحث:

التثبيت هو مرحلة من المعالجة التقويمية تعنى بالحفاظ على موضع الأسنان المعدّل تقويمياً باستخدام الحاصرات. تميل الأسنان للعودة إلى وضعها السابق (النكس) في حال غياب هذه المرحلة، ولذلك لابد من تطبيق إحدى طرائق التثبيت لكل مريض أجرى معالجة تقويمية لتجنب نكس المعالجة.

الغايات:

تقييم مدى فعالية طرائق مختلفة مستخدمة لتثبيت وضعية الأسنان بعد المعالجة التقويمية بالحاصرات.

خطة البحث:

تم البحث في المحركات التالية:

•   سجل التجارب السريرية التابع  لهيئة الصحة الفموية التابعة لـمنظمة كوكران Cochrane.
•    CENTRAL
•    MEDLINE
•     EMBASE
بالإضافة إلى تولي البحث اليدوي في مجلات التقويم تماشياً مع برنامج بحث منظمة Cochrane لصحة الفم. لم يتم التقيد بلغة معينة. و تم تحديد مؤلفي التجارب العشوائية المضبوطة و التواصل معهم لكشف التجارب غير المنشورة. تم إجراء البحث حتى تاريخ مايو/ آيار 2005.

معايير الاختيار:

أُجريت جميع التجارب السريرية العشوائية المضبوطة على الأطفال و البالغين الذين طبقت لديهم مثبتات أو تم عمل إجراءات إضافية لمنع نكس المعالجة بعد إزالة الحاصرات التقويمية.
كانت المحصلات هي: تقييم مدى تثبيت الأسنان، بقاء المثبات، التأثيرات الجانبية على صحة الفم و جودة الحياة.

جمع المعلومات و التحليل:

قام باحثان بمسح الدراسات المناسبة و تقييم جودة الطرائق المتبعة في التجارب و استخلاص المعلومات مرتين و بشكل مستقل. لم يكن من الممكن جمع نتائج أي من الدراسات مع بعضهما بحكم عدم وجود دراستين تقارنان نفس طرق التثبيت.

النتائج الرئيسية:

وافقت خمس تجارب معايير الاختيار، و قارنت طرقاً مختلفة من المعالجات:
•     مقارنة بين عملية قطع ألياف الرباط اللثوية الدائرية واستخدام جهاز تثبيت متحرك ( طوال الوقت) من جهة وبين استخدام جهاز تثبيت متحرك فقط (طوال الوقت) من جهة أخرى.
•    مقارنة بين عملية قطع ألياف الرباط اللثوية الدائرية واستخدام جهاز تثبيت متحرك (يوضع ليلاً ) من جهة وبين استخدام جهاز تثبيت متحرك (يوضع ليلاً ) فقط من جهة أخرى.
•    مقارنة بين جهاز Hawley المتحرك من جهة وبين المثبتة المغطية الشفافة من جهة أخرى.
•    مقارنة بين التثبيت بسلك متعدد الانحناءات مع التثبيت بشريط مقوى بالريبون و مثبت بالكومبوزت.
•    مقارنة بين ثلاثة أنواع من المثبتات الثابتة مع جهاز متحرك.
هناك دليل ضعيف لا يمكن اعتماده مستخلص من إحدى التجارب التي أظهرت  وجود زيادة ذات مغزى إحصائي في ثبات أسنان المنطقة الأمامية عند إجراء عملية قطع الألياف الرباطية اللثوية الدائرية  في الفكين السفلي (P<0.001) و العلوي (P<0.001). كان هناك أيضاً دليل على أن ثبات الأسنان عند استخدام جهاز Hawley المتحرك أسرع من ثباتها عند استخدام الجهاز المتحرك الشفاف بعد ثلاثة أشهر إلا أن الدليل ضعيف ولا يمكن الاعتماد عليه، وبشكل عام كانت جودة تقارير التجارب ضعيفة.

خاتمة المؤلفين:

تعتبر المعلومات المستخلصة من الأبحاث غير كافية لبناء قاعدة للممارسة السريرية فيما يخص تثبيت الأسنان حالياً. هناك حاجة ماسة لإجراء تجارب سريرية عشوائية مضبوطة عالية الجودة حول هذه المرحلة الحاسمة من المعالجة التقويمية.

There is not enough evidence about the effects of different types of retainers to keep teeth in position after the use of orthodontic braces.Retention is the phase of orthodontic treatment that attempts to keep teeth in the corrected positions after orthodontic (dental) braces. Without a phase of retention there is a tendency for the teeth to return to their initial position (relapse). To prevent relapse almost every patient who has orthodontic treatment will require some type of retention. There is a lack of robust evidence on which to base clinical practice in this area. This review found weak, unreliable evidence that a simple surgical procedure, combined with a retainer, is better than a retainer alone at keeping teeth in the corrected positions after orthodontic braces are removed. There is an urgent need for high quality randomised controlled trials in this crucial area of orthodontic practice.

Abstract

Background

Retention is the phase of orthodontic treatment that attempts to keep teeth in the corrected positions after treatment with orthodontic (dental) braces. Without a phase of retention there is a tendency for the teeth to return to their initial position (relapse). To prevent relapse almost every patient who has orthodontic treatment will require some type of retention.

Objectives

To evaluate the effectiveness of different retention strategies used to stabilise tooth position after orthodontic braces.

Search strategy

The Cochrane Oral Health Group’s Trials Register, CENTRAL, MEDLINE and EMBASE were searched. Handsearching of orthodontic journals was undertaken in keeping with the Cochrane Oral Health Group search programme. No language restrictions were applied. Authors of randomised controlled trials (RCTs) were identified and contacted to identify unpublished trials. Most recent search: May 2005.

Selection criteria

RCTs on children and adults, who have had retainers fitted or adjunctive procedures undertaken, following orthodontic treatment with braces to prevent relapse. The outcomes were: how well the teeth were stabilised, survival of retainers, adverse effects on oral health and quality of life.

Data collection and analysis

Screening of eligible studies, assessment of the methodological quality of the trials and data extraction were conducted in duplicate and independently by two review authors. As no two studies compared the same retention strategies (interventions) it was not possible to combine the results of any studies.

Main results

Five trials satisfied the inclusion criteria. These trials all compared different interventions: circumferential supracrestal fiberotomy (CSF) combined with full-time removable retainer versus a full-time removable retainer alone; CSF combined with a nights-only removable retainer versus a nights-only removable retainer alone; removable Hawley retainer versus a clear overlay retainer; multistrand wire retainer versus a ribbon-reinforced resin bonded retainer; and three types of fixed retainers versus a removable retainer. There was weak unreliable evidence, based on data from one trial, that there was a statistically significant increase in stability in both the mandibular (lower) (P < 0.001) and maxillary (upper) anterior segments (P < 0.001) when the CSF was used, compared with when it was not used. There was also weak, unreliable evidence that teeth settle quicker with a Hawley retainer than with a clear overlay retainer after 3 months. The quality of the trial reports was generally poor.

Authors’ conclusions

There are insufficient research data on which to base our clinical practice on retention at present. There is an urgent need for high quality randomised controlled trials in this crucial area of orthodontic practice.
